Vision, hearing, blood pressure, urinalysis dipstick, and a physical check against the FMCSA standard. 15 to 20 minutes a driver.

Pass and you're certified on the spot. We transmit the result to FMCSA and TX DPS that day — your card lives in the state system, not just in your glovebox.
The exam room is set up — and it travels.
Blood-pressure cuff, scale, vision screener, audiometry for the hearing check, urinalysis dipsticks, stethoscope, otoscope, reflex hammer. It all lives at the clinic, and it all packs into a case when we run a yard day — along with a laptop and hotspot to file on the spot, because plenty of yards out here don't have wifi.
All we need is a quiet room.
The hearing test is a whisper check at five feet, and a shop floor is too loud to pass anyone fairly. A spare office or a breakroom with the door shut does the job. Add a restroom for the urinalysis and that's the entire facility requirement.

No. The urinalysis in the physical is a health dipstick — protein, blood, sugar. A DOT drug screen is a separate, regulated collection, and we do those too — $50 added to an exam in the same visit.
Most issues are fixable — blood pressure, uncorrected vision. We'll tell the driver exactly what to bring back, and recheck. Nobody gets a surprise.
